Because eating less and training more only works if the hormonal system allows it. Chronically high cortisol, resistant insulin, a sluggish thyroid and bad sleep lock fat burning from the inside. It is not lack of willpower, it is a metabolism running in defense mode.
If diet and training were enough, you would have lost the weight already. The "eat less, burn more" math ignores who decides what the body does with energy: the hormones. And a body under chronic stress, sleeping badly and running high insulin receives the diet as a threat, not as a plan.
Defense mode
- •High cortisol steers fat storage to the abdominal region and increases hunger for calorie-dense food. It is the hormone saying "hard times, save everything".
- •Resistant insulin keeps the fat-burning door locked, even while eating little. Normal fasting glucose does not rule this out: insulin rises years before glucose does.
- •A sluggish thyroid lowers resting energy expenditure. TSH and free T4 tell the start of the story; in selected cases, T3 completes the picture.
- •Short or shallow sleep dysregulates the next day's hunger and satiety hormones: more appetite, less control, less energy to train.
Why "just eat less" makes it worse
Aggressive restriction in a body in defense mode raises cortisol even further and drops T3. The metabolism slows to fit the scarcity, the weight stalls, the yo-yo comes and so does the guilt. The person thinks they failed the diet. It was the diet that failed the physiology.
What to investigate before the next diet
Fasting insulin alongside glucose and A1C, TSH and free T4 (T3 in selected cases), late-night salivary cortisol when indicated, and, depending on the case, estradiol, progesterone and testosterone, with sleep taken seriously as part of the treatment. Sustainable weight loss does not start on the plate. It starts in the system that decides what the plate becomes.

The answer is not one more isolated lab test. It is reading the system.
Medicine is one. The body does not divide itself into specialties: the model does. When each symptom goes to a different office and each doctor sees one piece, you spend years circling between referrals, holding reports and missing answers. In that back-and-forth, you lose time and health.
Precise diagnosis and treatment come from the opposite path: understanding everything and going deep into one patient. Sleep, hormones, weight, energy and mind interpreted together, because that is how the body works.
