I was that patient. I went through dozens of doctors' offices until I understood that no one was looking at the whole system.
My clinical focus: studying how hormones, medications, vitamins and minerals interact in one body, and turning that individual study into one plan per person.

My Journey
I was the doctor who couldn't take care of himself.
160 kg (352 lb). Chronic insomnia. Binge eating. Anxiety that paralyzed me. I ate to stay awake and worked so I wouldn't have to think. I was doing everything mass medicine teaches: sacrifice, effort, total dedication. And my body started collecting the bill.
I looked for help the way everyone does. I went through four Brazilian states looking for answers. Nutrition physicians. Endocrinologists. Bariatric surgeons. Psychiatrists. Dozens of dietitians. Each one treated a piece. No one saw the whole.
And I kept going. Night shifts, public clinics, whatever came up. Until the night that changed everything.
I was working in two small towns. I finished a shift in Coelho Neto and chained another 24 hours in Duque Bacelar, a town of 10,000 people, in a hospital with no defibrillator. It was my third night without sleep.
I broke into a cold sweat. My heart was racing, out of rhythm. I saw my last patient and went to the on-call room. I couldn't leave: I was the only doctor there. I lay down thinking I was going to die right there.
That year, three colleagues had suffered heart attacks in Coelho Neto, where the defibrillator was broken. One of them died. His son studied medicine with me.
In that on-call room, alone, I understood: the next one could be me.
It was there that I had to admit something that still hurts to say:
"If healing others means getting sicker myself, then the model is wrong."
I stopped. I studied from scratch. I connected what no one was connecting: gut, hormones, sleep, mind. I treated it as a system, not as loose parts. My own body was the first case.
I lost 55 kg (121 lb). I sleep through the night again. Anxiety stopped running my life. I came back to life.

Investigation, Not Guesswork
It's the most common sentence I hear on day one. And it's almost always only half true, because no one ordered the tests that matter.
Most doctors order 4 or 5 basic items. I order between 15 and 25. It's not overkill. It's the minimum for someone to look at you as a whole.
Most doctors only order glucose. But the problem shows up much earlier: in insulin and A1C.

One TSH inside the range is not enough. The number is read together with the symptoms, and T3 comes in when the picture calls for it.

What happens when no one looks
Patient, 27 years old. Severe depression. He had already been through a psychiatrist, a general practitioner, an endocrinologist. Antidepressants, adjustments, more antidepressants. Nothing.
No one ordered total and free testosterone. Because "he's too young to have a hormonal problem".
I did. Total testosterone: 27, far below the minimum expected for a man his age. Alongside it: B12 near the bottom. Iron on the floor.
Before calling it treatment-resistant depression, someone had to look at the body: nutrients and hormone on the floor. No one had looked.

A large share of the body's serotonin is made in the gut, and gut and brain talk to each other constantly. When the gut is inflamed, mood, appetite and sleep tend to feel it too.
Chronic anxiety raises cortisol, which disrupts insulin and thyroid, locking weight loss even with a perfect diet.
Unbalanced hormones destroy sleep quality. Without deep sleep, there is no recovery, energy or focus.
